We encourage you to participate fully in your
medical care. One way to do this is to protect your health by
taking good care of yourself. Another is to be a well-informed
patient. We believe that well-informed patients make our best
patients. We are delighted to answer any questions you may have
or provide you with up-to-date medical information.
We believe that this participatory form of healthcare
builds a partnership between the patient and doctor that best
serves you, the patient. We hope you will do all you can to
keep yourself well. And, we promise to do our part to help you

New Patients
As a new patient, we will ask that you fill out a brief questionnaire
for our medical records. Your answers will enable us to give
you a complete examination.
Your Examination
Your examination takes place in a modern fully equipped medical
setting. A highly trained medical staff member who will make
you comfortable will escort you. They will conduct a brief interview
about your past health and discuss with you your current discomfort.
Dr. Adelglass will review the information obtained by our medical
staff, examine you, and after evaluation of the findings, he
will then explain to you the best procedure to follow to minimize
your discomfort.
About Children
Children are excellent patients if parents help in preparing
them for an office visit. The visit should be treated as a part
of the routine in his or her young life. Every effort is made
to avoid pain. Do not bring other children to the examination
if at all possible. A parent of guardian must accompany children
under the age of 18.

Areas of Expertise
Allergy
Medical treatment of inhalant allergies affecting the nose and
sinuses. Allergy testing can be performed by either a simple
blood test or by conventional skin testing.
Pediatric Otolaryngology
Medical and surgical treatment of diseases in the ears, nose
and throat in children. This includes tonsil, adenoid, and ear

Fees and Insurance
EXPLANATION OF PAYMENT AND INSURANCE FILING PROCEDURES
Our office is pleased to assist you in filing
claims with your insurance company for reimbursement of your
expenses. We will wait ninety (90) days for your insurance company
to pay your claim. If they deny your claim and/or do not pay
your claim for any reason, any remaining balance must be paid
by you within thirty (30) days. We will not fight your insurance
company for payment. We file insurance for you as a courtesy.
* It is the patient's responsibility to pay
any deductible and co-payments at the time services are rendered.
* Our office NEVER guarantees
that your insurance will pay. We will make every attempt at
the beginning of your health care to receive verification of
your policy benefits. However, if for some reason your insurance
claim is denied, you are responsible for the amount due on your
account immediately.
* We recommend that you also contact your insurance
company to verify the ENT and Allergy coverage on you policy.
* We will not file insurance for patients that
do not live in Texas. We will provide you with receipt of all
services rendered.
* If injections, allergy testing, ear cleaning,
or other in-office procedures are recommended for you treatment;
please be aware that they may be considered surgical procedures
and will be applied to your deductible.
* Our office will not file with seconday insurance
policy unless Medicare is your primary.

Calling Our Office
The receptionist or nurse will receive calls. They have been
trained to answer most questions. If the doctor's opinion is
required, you will be advised to expect a return call. Please
remember that many diseases have similar symptoms; for this
reason, most requests will require an office visit. Medication
cannot be prescribed for a medical problem that has not been
recently evaluated. For refills of medication, please have your
pharmacy call the office only during regular office hours. Please
always keep your pharmacy phone number handy.

ABOUT OUR DOCTORS
Jeffrey Adelglass, M.D.
Dr. Jeffrey Adelglass is the founder of Skintastic Medical and
Surgical Skin Rejuvenation Centers. He is a Fellow of the American
Academy of Cosmetic Surgery, a member of the American Academy
of Facial Plastic and Reconstructive Surgery, an Associate Professor
of Otolaryngology at the University of Texas Southwestern Medical
School and currently serves on the Medicis Board (makers of
Restylane). Dr. Adelglass was also chosen by Allergan to serve
on the National Education Committee, because of his many years
of experience, to teach others his latest and safest ways of
administering Botox.
Dr. Adelglass is also a prominent researcher,
and founder of a renowned medical research company Research
Across America, and a frequent lecturer. He is regularly
featured on local television programs and provides the daily
pollen count for local television and radio stations.
Dr. Gregory Roberts joined Skintastic in May, 2005, and has
been practicing since 1998. He is board certified by the American
Board of Otolaryngology as well as the American Board of Facial
Plastic and Reconstructive Surgery. He received his medical
degree from the University of Texas Health and Science Center
in San Antonio, Texas in 1992 where he was elected into the
Alpha Omega Alpha Honor Medical Society. He completed five years
of residency in Otolaryngology Head and Neck Surgery at the
University of Missouri, Colombia. He then completed a fellowship
in Facial Plastic and Reconstructive Surgery with Keith LaFerriere,
M.D. A leader in the field of facial plastic surgery and past
president of the Academy of Facial Plastic and Reconstructive
Surgery. Dr. Roberts also has training as a Dentist.
